The SAP team gathers data such as grades, attendance, and discipline referrals along with classroom teacher observations to determine if there is a significant problem. Each SAP team is typically composed of counselors, teachers, a nurse, home and school vistor and administrators.
An interview with the student may be scheduled.
This screening results in a plan of action. The student is often referred to resources such as counseling, mentoring, tutorials, etc...
Some students may receive a referral for an evaluation for possible special education services.
At times the problem is beyond the scope of school, and the parents are given information about community-based behavioral health assessments.
