Community Service Verification Forms

In order to receive credit for your community service you must complete a log sheet and a reflection sheet.  Click on the links below to download these forms. Forms should be handed in by September 15th for any summer volunteer work, and at the end of each semester or year end for all other hours completed. Forms are to be handed into the Counseling Office.
