The Nurse's Office is open from
7:20 AM till 2:20 PM every school day.
Student Emergency Forms
A Student Emergency Form (pink) will be sent home with your
student the first day of school. Please complete all sections of
the form including a parent signature and return the form by
the first week of school to their homeroom teacher.
It is of the utmost importance that you complete these
forms so that we, as a school system, are able to contact you
with regards to your child.
Physical Education Excuses
If your child is ill or has a physical impediment that prevents him/her from participating in physical education a parental note is acceptable for one class period following the injury. If your child needs to extend that excuse for a longer period of time you must provide the school with a physician's note with the diagnosis and length of time that the child will be unable to participate in physical education.
Medication
As per District Policy, students are not to bring any medication to school without a physician's order and parent note.
Over -the counter medications listed on the Student Emergency Form are administerd with permission from the school physician and parent approval as indicated by parent signature and checking "YES" on the medication. Any other over-the-counter medication will require a physician and parent note.

Early Release Due to Medical Concerns:
If your child becomes ill/injured during the school day this procedure will be followed:
1. The student is to report to the nurse's office to be assessed for dismissal from school. Student's are not permitted to contact their parent prior to being assessed.
2. The Parent/guardian will be called first. A student will not be released to anyone that is not listed on the emergency contact list unless, the parent calls and states name of the individual that will be picking up student from school.
3. Students will sign out in the attendance office. Parents may be asked to come into school and sign out the student.
